To satisfy the requirements of our clinical affiliates
and accreditation body (ARC-PA), it is requested that all
students within the MUPA program obtain and have on file,
with the Methodist University
Student Health Services, proof of health screening
and immunization status consistent with CDC recommendations
for adults and healthcare workers.
At the time of matriculation, all students are required
to have completed and have on file at the student health
services the following:
- a health status form
completed prior to matriculation by a licensed medical
practitioner which verifies that a physical examination
has been done and that the student is free of communicable
disease
- an immunization record
documenting up to date immunizations and where applicable,
immunization series in process; the immunization record
should include the date, and the name and address of the
facility administering the following immunization:
- Hepatitis B vaccine (series complete or in process,
or results of antibody titer)
- PPD results (or Chest X-Ray as applicable)
- MMR and Td/Tdap
- Varicella
Evidence of immunity to varicella in adults includes
any of the following:
- documentation of completed varicella vaccination
- history of varicella based on diagnosis or verification
of varicella by a healthcare provider,
- history of herpes zoster based on healthcare
provider diagnosis, or
- laboratory evidence of immunity or laboratory
confirmation of disease.
Other vaccinations - such as the influenza, pneumococcal,
meningococcal, Hepatitis A, and HPV, are not required. Entering
students may use their own discretion and the advice of
his/her clinician concerning these vaccinations.
Students are required to update
and maintain current health and immunization records
with Student Health Services. A student is also required
to maintain a current copy
of his/her immunization record on file with the PA program
administrative office and carry a copy of his/her immunization
record when attending clinical activities (during the didactic
and clinical phase of training).
